First off, Deep wounds will better for your confidence than Bloodcraze, Blood Craze is pathetic healing, the only time it will really save your life in a 5 man is if the healer ninja-left group. Deep wounds will end the fight quicker Saving the healer mana and minimizing the time you're actually taking damage. If you have a healer Deep wounds is the better survival talent.
Secondly, you want max points in Incite AND shield Specialization. Heroic strike should be making up around 35% of your damage in an infinite rage scenario (i.e. making it your biggest single source of damage). If you're not in an infinite rage scenario you need to engineer an infinite rage scenario. Shield Specialization is your best way of doing that. Incite is your only talent that affects heroic strike, and 15% chance to crit, and auto crit after that is a crap tonne of extra damage.
Gag Order: You do not need this, it's a crutch, sure it can make pulls a little easier but its really a PVP talent. Like Blood craze.
Imp Rev or Thunderstruck, Thunderstruck is technically better damage but Imp Rev is much more rage efficient. Pick one. If you don't pick Imp Rev take Rev off your bars.
Your single target rotation is wrong, its more a priory system.
1. Shield slam: if this is not on CD, put it on CD.
2. Make sure Rend is UP: If it isn't on the target use rend, If it has less than 3 seconds left refresh it via thunderclap
3. Shorten the CD of Shield Slam: if SS has more than 1 GCD left on its CD use Dev (or Rev) to proc SnB.
4. Trucking Time: this is the best GCD to use your Trucks, Shockwave and Concussion Blow. the former does more damage, the latter more threat.
YOur AoE priority should be
1. Spread Rend
2. Shockwave
3. Thunderclap
4. Revenge (if specced imp rev)
5. Shield Slam
6. Conc Blow
7. Devastate
Obviously you shuld also be keeping the demo shout buff up, anytime you're survival isn't guaranteed. Using the appropriate rage dump at above 50 rage, and Inner rage above 85 rage, as well as spell reflecting interrupting and heroic leaping.
Get Glyph of Demo shout. Makes keeping it up SO much easier. You want to try and keep your dodge and parry rating closer together for maximum Combat table coverage. Finish off your Profs. neither are that great for tanking but any bonus is nice to have. Battle shout is more important to your survival than Commanding shout if you ever have to choose between the two, at least until fully CTC capped.
EDIT: A little further information on Revenge. Don't get me wrong I love Revenge. If their was a campaign to buff revenge i'd there throwing numbers at the opposition. The problem is that it's base damage is poor compared to Devastate, and while Rev does scale better off your AP than Dev, and you do have a lot of AP, Dev also scales off your weapon damage, you need quite alot of AP before Rev out does Dev, and while this is complicaed a little by being in the situation that rev may allow a few more Heroic stikes until you reach the point where most of your gear is about a half tier ahead of your weapon Rev just never catches up to dev in terms of Damage Per Execute thanks to hits higher crit chance.
