Threat Rotation

I've been looking for a thread on a threat rotation but haven't found one that specifically outlines which abilities we should be using when. I had a couple of questions.

1) What abilities should we prioritize?
2) Should we rend if we don't have a bleed debuff in the raid?
3) Concussive Blow and shockwave are still really good right?
4) What's this I hear about revenge being kind of bleh?
5) Deep wounds talent worth it?
