there are also some ingame addons that may help newer rail leaders figure out what went wrong...
ensida fails -- brutal account of each toon's failure to avoid avoidable damage. maybe the tank dies because the healers went OOM sending a little extra help to player X.
utopia--the uptime window gives a good overview of buffs and debuffs applied during a fight. useful for finding if someone forgets to reapply buff/debuff...
@ cruel -- yes there is an addon for an addon that replicates what skada does internally. which do you think will break first? or were you trying to be ... well cruel...

and both of them Skada & Recount use Ace and Ace is breakable. =)

I am absolutely amazed at how few people at least on my server, don't know the information that you can get from Recount. I find it a very useful tool in raids, instances and even on Battle grounds though its mostly how did I die and how can I prevent that from happening again.
