Question about addons.

I am looking to reset all my addons. I have so many and its gets so confusing at times. I was looking around the site for some kinda addon sheet that People here use. When I came across this.

Now, If I could be told how to make my screen look like that. I would greatly appreciate it. I am kinda slow with these kinda things also, so if it wouldnt be to big of a hassle. Please give me like step by step instuctions. Thank you.
