When i Offtank or even when I main tank I never reapply diseases i always use pest ot refresh them to allow for more HS/DS depending on role and any other variables.

Speaking as a blood tank, he's likely using HS for the cleave component for AoE tanking and switching targets often enough to hit as many as he can between his DnD's and BB's.
