Your low health is partially a result of low level gear, and largely a result of trinket choice. Get rid of the dodge trinket for Black Heart from reg ToC 5-man, and your health (and survival) will improve.

Also, you did not stack a lot of 30 stam gems. I see a 20 dodge gem in your helm and two defense gems in your boots. Getting the proper helm and shoulder enchants would be a big help in this regard - +37 stam and +20 def to helm, and +30 stam +15 resil to shoulders (PvP enchant, bought for 10k honor). These two enchants would make you uncrittable without the two def gems (15 resil reduces the def minimum for raiding to 536). You also should replace that helm gem with +10 agi/+15 stam, which is far superior for survival.

You're also using a DPS ring. Aim for either Mark of the Relentless (H ToC 5 man) or Signet of the Impregnable Fortress (Valor badges). Or you could run a few ICC bosses and get the Ashen Verdict rep ring.

Your weapon is missing an enchant. I'd just toss something cheap on there since it's not such a great weapon, probably just +26 agility (or the lesser agility one, I forget the number). Don't waste the mats on Mongoose or Blood Draining.

Your spec is a large part of your aggro problem. (I'm assuming you looked up the 96969 rotation and are using it for single-target)

Because our Judgements, Seals, and Consecrate all scale with SP in addition to AP, Touched by the Light is one of the best threat talents you can spec into. Judgements of the Just and Vindication are required for any raider and recommended to non-raiders (20% slow to incoming melee damage and a Demo Shout effect, both will reduce your incoming damage by a lot), Divinity is a very weak survival talent (1% healing per talent point is not worth it), and you chose Seals of the Pure over Crusade, which is a bad decision. Crusade affects almost every single mob and boss, making it a flat 6% increase to damage rather than just boosting your SoV and Judgement damage.

You also should replace either Righteous Defense or Judgement glyph with Seal of Vengeance - free 10 expertise is awesome.

Try this spec: http://www.wowhead.com/talent#sZV0tAbuMusIufdxfMzbc:GpZmc

It also gives you access to Seal of Command, which is amazing for AoE tanking (most 5-mans and a couple raid fights like Lootship or Dreamwalker). Pursuit of Justice is nice utility as well (don't forget to change boot enchants!).

I agree with a most of the things that Harmacy said. Your spec could use some work, for mine when I was learning about tanking I used http://modenstudios.com/invisusira/index.php?option=com_content&task=view&id=110&Itemid=72 . The cookie cutter at the bottem is what I used and it hasnt let me down yet. Plus its a good page to have it explained why some talents are good and some are bad. You should also check out http://maintankadin.failsafedesign.com/ . It has pretty much all of the information you would need to get right on track for your rotation and the theorycrafting behind it.

If you want you can check out my spec http://www.wowarmory.com/character-sheet.xml?r=Bladefist&cn=Ultimaspike&gn=Soldiers+of+God . While some people will debate the best/worst glyphs to use the ones I have on I stand by, havn't ran into a problem yet that would require me to change it, but thats just my opinion.

I did notice too that you are missing the Faction enchants and you should really grind rep with them because they can help immensly, especially Ardent Crusade and Sons of Hodir.
