You outgear me and are ahead in progression, but since no one has responded yet, will post a few observations.

You appear to be adequately geared for the fights you mentioned. Are you sure it's not a healing problem? Have you loaded your combat logs into worldoflogs.com or wowwebstats.com and reviewed the fights closely to see if you're getting healed frequently enough?

Looks like you're already heavily gemmed for stamina. Only gem issue I see is minor - your bracers have a crit/stam gem instead of a avoidance/exp/hit/stam gem.

Your hit and expertise are below caps, but I assume raid/food/flask buffs compensate?

Your Blood spec looks pretty normal, but are you sure your frost spec is kosher? Usually Frost dk tanks don't bother getting Morbidity or Icy Talons, favoring Unbreakable Armor and Bladed Armor instead. Icy Talons gives single-target threat, but also more parry-haste, while Bladed Armor buffs your autoattacks, special strikes, spells, and diseases. A tank w/ 25k armor in Frost Presence will get almost 700AP from 5/5 Bladed Armor. And UA is actually a decent tank cd post patch 3.2.2.
